The following DepEd School Forms Checking Report Templates (SFCRs) are attachments from DO. No. 11, S. 2018 also known as “GUIDELINES ON THE PREPARATION AND CHECKING OF SCHOOL FORMS”. The Department of Education (DepEd) issues the enclosed Guidelines on the Preparation and Checking of School Forms for the preparation, updating, and evaluation of school forms in basic education. The mechanisms and simplified procedures prescribed by these guidelines are anchored on the principles of accountability, accuracy, and reliability of data, and efficiency. They shall reduce the time and effort of school personnel spent on clerical tasks and records management without compromising the accuracy of the learners’ information and the quality of school forms.

The Department of Education (DepEd) issued the standard process and protocols for the preparation, evaluation, and updating of school forms conducted at the end of every school year. This is to (1) provide a reliable assurance mechanism of learner information, (2) ensure the quality and timeliness of school reports and (3) reduce the resources spent on clerical and records management. Below are the downloadable softcopies of the DepEd School Forms Checking Report (SFCRs Templates per Adviser, District, and Division). How to use School Forms Checking Report Templates (SFCRs)
Read the simple instruction below regarding the use of School Forms Checking Report Templates (SFCRs).
School Checking Committee
Sundin ang suggested minium no.s of classes per category or ang percentage estimate which ever is higher.
Read more about: DO.
